He is survived by : his wife Vicki Poulos; his sons, Chris (Christina), Brett (Courtney) and Derrick (Brittni); and his grandchildren, Harper, Casey and Decker. He is also survived by numerous other family members and friends.

In lieu of flowers, the family requests that donations be made to the Optimist Football Program in Brad's honor.
