He was predeceased by: his father Charley Leon Forman. He is survived by: his wife Heather J. Forman (Browning); his son Dylan; his stepchildren, Jacob Kellar, Chase Kellar, Zoey Kellar and Savanna Miner; his grandson Grayson John-Paul Kellar; his mother Lydia Helen Forman; his brother Barry Forman; his sibling's children, Brittany, Angela, Dustin and Jacob; and his pet Beaux.

A celebration of life will be held on Saturday, July 26th 2025 from 1:00 PM to 3:00 PM.
