He was predeceased by: his parents, Louis Cook and Peggy Cook; and his daughter Pamela Kologinczak. He is survived by: his wife Marsha Cook; his children, Sherri Conran, Todd Cook, Lori Brown (Kevin Dixon), Bruce Milliken and Karen Milliken; his brothers, Dwight and Earnest Cook; his sisters-in-law, Debbie and Alice; his sister Judy Day; and his grandchildren, Michelle, Tiffany Sarah, Chelsi, Courtney, Jolie, Tiffany Le'anne, Cameron, Dalton, Nathan, Brandon, Hunter and Connor. He is also survived by grandfather, as do his 14 great-grandchildren and 2 great-great-grandchildren.
