He was predeceased by: his parents, Alex Mikolajczyk and Clara Mikolajczyk; his siblings, Alexine, Allen and Alvin Mikolajczyk; his nephew Kermit Mikolajczyk; and his sister-in-law Denise Greiner Mikolajczyk.
He is survived by: his sons, Dustin (Renatte), Brandon (Caitlin) and Sheldon Mikolajczyk; the mother of his children Pam Walls Mikolajczyk; his grandchildren, Annabella, Athena, Radley and Kollins; his siblings, Clarence (Cindy), Albert "Mik" (Shanti) and Clarine Boeck (Wally); and his friend Leland Carver. He is also survived by numerous nieces, nephews, cousins, and lifelong friends.
