He was predeceased by: his mother Betty Blair. He is survived by: his wife Emma Hodges Blair; his children, Michael Dean Blair Jr., Kyle Brandon Blair, Chase Ryan Blair, Erin Blair Prochaska and Matthew David Blair; and his sister Cathie Lynn Neuhaus. He is also survived by nine grandchildren.
