He was predeceased by: his parents, John William, Sr. and Doris Samuelson (Wynne); and his parents-in-law, Ralph and Eleane Hook. He is survived by: his wife Dawn Samuelson; his sons, John Chesworth and William Chesworth; his brothers, Robert Wynne and Wynne Samuelson; and his siblings-in-law, Ralph Hook and Debbie Walker. He is also survived by numerous aunts, uncles, and grandchildren.
