He is survived by: his parents, John (Deenna Mayo) and Jack (Jan Burk); his son Declan Carter Mayo; his sibling Corey Mayo (Dena); his nieces and nephews, Alexus (Seth) and Dalton; his brother Johnny Mayo (Natasha); his nieces and nephews, Kaidyn and Austin; his siblings, Austin Burk (Crystal) and Tanya Tolleson; his nieces and nephews, Alyssa (Arturo), Tristyn and Wyatt; his brother Jake Staats (Brooke); his nieces and nephews, Landon, Riley, Bradley and Hunter; his siblings, Grant Burk and Lauren Staats; his nieces and nephews, Allie and Seth; his grandmother Nellie Landree; his aunts and uncles, Judy (Rick May), Cindy Daily, Dan (Debbie Temple), Randal (Dale Ward) and Jason Landree; and his cousins, Mark, Chris, Deanne, Dane, Justin, Wesley, Stevie and Amy.
