He was predeceased by: his parents, Robert Middlebrook and Albina Middlebrook (Koval); his siblings, Bobby Middlebrook (Barbara) and Rose Swank (Jerry); his niece Lisa Middlebrook; and his nephew Eddie Middlebrook Sr..
He is survived by: his brother Ervin Middlebrook Sr. (Marjorie) of Chalk Bluff; and his nieces and nephews, Marlu Duncan (Jay), Robert Middlebrook, Marva Simonette (Tracy), Ervin Middlebrook Jr. (Kristy), Jeannie Frost (Melvin) and Kenneth Swank (Dianne). He is also survived by many great nieces and nephews, other relatives and friends.
In lieu of flowers, memorials may be made to Closner Cemetery Association or West Little League Baseball Association.
