He was predeceased by: his parents, Charles Moroni Hafen and Mary Evelyn Voyles Hafen; his brother Richard Lee Hafen; and his grandparents, Weston, Fern Hafen, Hyrum and Mildred Voyles.
He is survived by: his siblings, Emily, James, Shannon Campbell (Kay) and Heather Giammona (Ciro); his children, Jerry Lynn Hafen Jr., Crystal Robynn Hafen, Rosalee Angel Hafen Ladd, Charles Alexander Hafen Rubio and Jason Scott Hafen Rubio; and his grandchildren, Braylon, Breezie, Porsche, Bryleigh, Jack, Luxston, Vallie, Kaizlie, Brazen, Liam, Raelynn, Wiley, Jasper, Auren, Margaux and Dahlie. He is also survived by many uncles, aunts, cousins, nieces, and nephews.
