He was predeceased by: his sister Christy; and his father-in-law David Ellingsworth. He is survived by: his daughters, Alexis, Alesha, Arianna and Annalia; his wife Jamie Ann Ellingsworth; his parents, Raeone (Christensen) and Bradley; and his siblings, Tonya Collins (Roy), Ryan (Angela), Rodd (Julie), Eric (Emma), Kimberley Rivers (Jonathan) and Ashley Bennett (John). He is also survived by 30 nieces and nephews, and 1 great niece.
All attendees are required to wear face masks and practice social distancing measures in accordance with the Davis County Health Department and CDC guidelines.
