He was predeceased by: his parents, Nicholas Phillip and Pollyanna Scholty Kozak; his sister Rickie Tea; and his friend Lucy Lou. He is survived by: his children, Phillip Dean and Paula Kim; and his wife Kayleen Braisford. He is also survived by Son, Daughter, 12 Grandchildren and 19 Great Grandchildren.
