He was predeceased by: his wife Shannon; his father Grant Cook; his in-laws, Wesley and Della Barber; his children, Heath Cook and Vickie Crozier (Tim); and his great grandchildren, Tinsley and Cohen Dodge.
He is survived by: his mother Carol Lynn Cook; his siblings, Judy Chlarson (Ted), Janice Richardson (Jerry) and Andy Cook (Cynthia); his siblings-in-law, Linda St. Clair, Carol Southworth and Roy Cottam; his nieces and nephews, Holly Heath (David) and Jerie Lynn Dodge (Troy); his son-in-law Tim Crozier; and also Jerry. He is also survived by 11 grandchildren and 23 great-grandchildren.
Visitation was held on Tuesday, May 27th 2025 from 10:00 AM to 10:30 AM at the Gillies Funeral Chapel (634 E 200 S, Brigham City, UT 84302). A funeral service was held on Tuesday, May 27th 2025 at 11:00 AM at the same location.
