He is survived by: his mother Linda Leavitt; his wife Shelly (Gilbert); his children, Chelsey Dorber (Tom), Dallas and Brantley; his grandchildren, Tommy and Scotlynn; and his siblings, Mindy Whiteley (Wendell), Kerry Harris (Dean), Kori Nelson (Braydy), Wade (Amy) and Aaron (Tanya). He is also survived by relatives and friends.
In lieu of flowers, the family requests donations be made in Mark's memory to the PKD Foundation, a cause that was close to his heart.
