He was loved and cherished by many people including: his parents, Phillip Boltz and Georgia Faye Boltz of Lehi, Utah.; his wife Carrie; his children, Shalyn Boltz, Amanda Boltz-Prue (Ron), Tyson Boltz (Holly) and Mckenzie Fawcett (Tyson); and his siblings, Phillip Boltz and Patricia Jakeman. He was also cherished by 11 grandchildren, 1 great-grandchild.
