He is survived by: his mother Thelma Falslev (Rasmussen) of Logan, UT; his children, Savanna Berlyn and Jax Ephraim Falslev of Providence, UT; and his stepdaughter Jade Stott of St George, UT. He is also survived by brothers, and one older sister, and multiple nieces and nephews.
