She was predeceased by: her father Clark Boyden Slade. She is survived by: her mother Wanda Chadburn; her siblings, Georgia, Vicki and Doug; and her son Jason. She is also survived by her four grandchildren.
Viewing was held on Saturday, May 31st 2025 from 10:00 AM to 11:00 AM at the Memorial Mountain View Mortuary, Cemetery (3115 Bengal Blvd, Cottonwood Heights, UT 84121). A funeral service was held on Saturday, May 31st 2025 from 11:00 AM to 12:00 PM at the same location.
