He is survived by: his children, Camille Ortiz, Michael and Matt Henderson; his grandchildren, Ruben Ortiz, Talia Ortiz, Dakota, Wyatt Wallace and Christian Henderson; and his sister Melody Clark (Scott).
His passing in Farr West, Utah, after a courageous battle with Alzheimer's and Dementia, has left an unfillable void in the lives of his beloved family and friends.
