She was predeceased by: her parents, Thomas Ford and Hazel Wood; her husband Richard Leavitt; her son-in-law Harold; her brother Ray; her half-sisters, Alta, Leah, Luella, Erma, Bertha and Jeanette; and her siblings-in-law, Richard, Horton, Wells, Grant, Lee, Maxine, Delora and Colleen.
She is survived by: her children, Rick (Jolene) and Leisa; her grandchildren, Tiffany Mizell (Aaron), David Leavitt (Hailey), Steven Reichard, Rebecca Graham (Anthony), Trevor Leavitt (Wendy), Matthew Reichard, Jordan Reichard (Jessica) and Taylor Reichard; and her sisters, NaDean Steffen and Elizabeth Hess.
Viewing will be held on Wednesday, November 4th 2026 from 6:00 PM to 8:00 PM and on Thursday, November 5th 2026 from 9:30 AM to 10:30 AM at the Myers Mortuary (845 Washington Blvd, Ogden, UT 84404). A funeral service will be held on Thursday, November 5th 2026 at 10:30 AM, at 11:00 AM and at 12:15 PM at the Myers Mortuary - Ogden (845 Washington Blvd, Ogden, UT 84404).
