He was predeceased by: his parents, Ervin Cook and Lillian Ruth Betts Cook; his siblings, Terry Cook, Oscar Cook and Anita Berlin; and his grandson Matthew Cook.
He is survived by: his wife Carole Irene Crismon Cook; and his children, Charles Cook (Marilee), Don Cook (JoLin), Camille Lund (Lorin), Rosanne Hogge (Jeff), Rebecca Hall (Joshua), Jennifer Ashby (Klay), Daniel Cook (Annette) and David Cook (Mayra). He is also survived by spouses; and his 27 great-grandchildren.
Viewing will be held on Friday, June 20th 2025 from 6:00 PM to 8:00 PM and on Saturday, June 21st 2025 from 9:30 AM to 11:30 AM at the LDS Church (1260 W 1150 N St, Provo, UT 84604). A funeral service will be held on Saturday, June 21st 2025 from 11:00 AM to 12:00 PM at the same location.
