He was predeceased by: his parents, Ervin Cook and Lillian Ruth Betts Cook; his siblings, Terry Jack Cook, Oscar Ervin Cook and Anita Ruth Cook Berlin; and his grandson Matthew Cook.
He is survived by: his wife Carole Irene Crismon Cook; and his children, Charles Cook (Marilee), Don Cook (JoLin), Camille Lund (Lorin), Rosanne Hogge (Jeff), Rebecca Hall (Joshua), Jennifer Ashby (Klay), Daniel Cook (Annette) and David Cook (Mayra). He is also survived by his 41 grandchildren and their spouses; and his 27 great-grandchildren.
