He was predeceased by : his parents, LaVon Forsyth and Martha Naomi Clark Forsyth; his stepmothers, Ruby Brinkerhoff Morrell Forsyth and Emma Baker Taylor Forsyth; and his siblings, Clayson LaVon Forsyth, Lowell Clark Forsyth, Terry Neil Forsyth, Brent M. Morrell, Montez Chidester, Grace Blackburn, Fay Nydegger, Shirley Smith and Arden P. Taylor.
He is survived by : his wife Melba Mae Ellett of Bicknell; his children, Marlin Forsyth (Mikell) of Centerville, Naomi Nixon (late Steven) of Cedar City, Mont Forsyth (Kelley) of Bicknell, Randy Forsyth (Tori) of Cedar City and LaVona Adams (Danny) of Cedar City; his siblings, Ward R. Forsyth (Judy) of Aurora, Karen Forsyth and Donald Forsyth (Flovon) of Bakerfield; and his stepsister Myrna Chappell (Dick) of Springville. He is also survived by 32 grandchildren; 84 great-grandchildren and 5 great-great-grandchildren.
