He was predeceased by: his grandparents, Garber, Kathy, Steve Madsen (Sherrie), Terry (Nancy), Sheldon (Janine) and Scruffy; his sister Amber; and his grandparents, Mel (Chad) and Sherry.
He is survived by: his daughters, Karizma Patience and Jewel'iahna Grace; his parents, Nicole Black and Steven "Greg" Madsen (Suzanne); his siblings, Ryan Anderson (Joanie), Trevor Anderson (Rebecca), Skylar Anderson, Dylan Madesen, Garrik Smith (Shani), Paige Krogh (Auston), Tiffany Lockwood (Brian), Lacee Embleton (Alan) and Samantha Madsen; and his friends, Chad, Glen, Tyler, Heather and Shyeanne. He is also survived by many aunts, uncles, nieces and nephews.
