He was predeceased by: his parents, Floyd Stephens and Laurel "Joy" Olds Stephens; and his sisters, Sharon Dunyon (Stephens) and Laurie Jill Stephens. He is survived by: his wife Elaine; his children, Jolyn Ann Stephens of Cedar City, Utah., Elizabeth Jane Stephens of Cedar City, Utah. and Michael Scott Stephens of Cedar City, Utah.; and his sister Tawnya Stephens.
