He was predeceased by: his cousin Hillary Hope. He is survived by: his parents, Cristi Ann Green and Todd Kimball Jacobsen; his siblings, Sarah (Adam), Daniel, Mary (Zachary) and David; and his siblings' children, Brent, Tay, Joseph and Lucy.
Viewing was held on Saturday, November 15th 2025 from 6:00 PM to 8:00 PM and at 8:00 PM at the Larkin Mortuary (260 E S Temple St, Salt Lake City, UT 84111). A memorial service will be held on Tuesday, November 18th 2025 from 12:30 PM to 2:30 PM at the Larkin Mortuary (2350 E 1300 S, Salt Lake City, UT 84108). A burial will be held on Tuesday, November 18th 2025 from 3:00 PM to 3:30 PM at the Salt Lake City Cemetery (200 N St E, Salt Lake City, UT 84103). A graveside service will be held on Tuesday, November 18th 2025 at 3:30 PM at the same location.
