He was predeceased by: his wife Connie Campbell; his parents, Frank Madsen and LaVee Madsen; his parents-in-law, Vere and Laverna Campbell; and his in-laws, Galen, Susan Campbell, Jared Campbell and Chiro Costanza.
He is survived by: his son Andrew; his daughter-in-law Tracy; his grandchildren, Hadley, Eleanor and Jack; his siblings, Sterling and Lynette Costanza; and his in-laws, Dirk, Kathy Horton, Peter and Angie Jensen. He is also survived by numerous nieces and nephews.
