She is survived by: her parents, William Hal Fowkes and Stella Fowkes (Titus) of Magna, Utah.; her husband Gary Chavez; her son Alex Chavez (Karly); her grandchildren, Kenzee and Mason; and her siblings, Lynn Bearnson, Dave Fowkes (Lindy), Bruce Fowkes (Karla) and Robyn Stam (Rick).
In lieu of flowers, Suzanne requested that donations be made to Huntsman Cancer Institute colon cancer research.
