She was predeceased by: her father Theodore Hayward; and her husband James Crook. She is survived by: her husband Melvin Ivie; her mother Maxine Murray DeVitt; her brother Murray (Elona); her children, Dennis, Danielle (Craig) and Suzanne (Scott); and also, George McMullen and James Crook. She is also survived by grandchildren and great-grandchildren.
Memorial donations may be made in her name to the Wounded Warriors Fund or Best Friends Animal Shelter-Kanab, Utah.
