We are sad to announce that on April 28, 2023, at the age of 59, Kurt Lewis Provost of Santaquin, Utah passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by: his parents, Jerry Provost and LouAnn Provost; his wife Susan Hickman; his children, Trent Provost, Kaylyn Curtis (Nick) and Taylor Provost (Gaby); his grandchildren, Homero, Vivian, Adriel, William, Sydney and Ivory; and his siblings, Jenice Allred (Kevin), Dan Provost (Janine), Wade Provost (Charline) and Amy Provost. He is also survived by many nieces and nephews.

In lieu of flowers, donations can be made to Primary Children's Hospital.
