She was predeceased by: her grandparents, David and Nancy Church; her uncle Gordon Ray Church; and her pet Maverick. She is survived by: her parents, Kevin Church and Karla Church; her husband Jakob Graves; her children, Piper Pearl, Remi Ray and Rafe Jakob; her sibling Mackenzie Smith (Tyler Smith); her nieces, Maci McGinnis and Sydnie Smith; and her nephew Max McGinnis (Parker Church). She is also survived by many extended family members and friends.
A funeral service will be held on Saturday, October 24th 2026 at 9:30 AM at the LDS Chapel. A celebration of life will be held on Saturday, October 24th 2026 at 11:00 AM at the Stake Center (1570 W 11400 S, South Jordan, UT 84095).
