Melissa Ann touched countless lives with warmth and kindness, leaving behind cherished memories that will forever echo in hearts. She was predeceased by: her mother Darlene Buttars Smith; her son Colten Clyde Frazier; and her grandparents, Ross, Louise Buttars, Hazel Smith and Asell Chugg Smith.
She is survived by: her children, Tayler Ann Frazier, Brock Andrew Frazier and Jake Dustin Frazier; her grandchildren, Yoselin Castillo (Yazzie), Nayeli Castillo (Neli), Remi Frazier, Ruger Frazier and Bowen Smith; her father Clyde Smith (Patty); and her siblings, Amy Johnson (Dave), Corey Smith (Liz), Dan Smith (Jenny), Mike Smith (Lizzy), Jenni Hansen (Skye) and Bug Allen (Blake). She is also survived by many nieces, nephews, and cousins.
Viewing was held on Friday, September 19th 2025 from 10:30 AM to 11:30 AM at the Lewiston 2nd Ward Chapel (10 S 1600 W, Lewiston, UT 84320). A funeral service was held on Friday, September 19th 2025 at 12:00 PM at the same location.
