He was predeceased by: his parents, Owen Lloyd and Donna Parker Cook; his significant other Judy Ann Bobchalk; his son Darin Andy Cook; his granddaughter Baylie Mae Larsen; and his brother Calvin Cook.
He is survived by: his children, Dean Cook (Leisa) of Vernal, Utah, Daniel Cook of Salem, Oregon, Lisa Larsen (Wayne) of West Jordan, Utah and Kristie Cook of West Jordan, Utah.; and his siblings, JoAnn Hardy of Apple Valley and Kay Cook (Judy) of West Jordan, Utah.. He is also survived by 13 grandchildren, 16 great-grandchildren.
