He was predeceased by: his parents, Junior Sullivan and Frances Sullivan; and his siblings, Johnny, David, Donald, Gale and Bea.
He is survived by: his wife Polly; his son Steven; his stepsons, Jimmy and Luke Cox; his siblings, Woody, Debra Phelps and Kathy Skeens; his friend Cathy Harrison; and his nieces, Trish Stanley and Jamie Smith. He is also survived by 9 grandchildren and 2 great grandchildren, several nieces and nephews.
