He was predeceased by: his parents, Mahlon Mayhew and Joyce Ayers Mayhew. He is survived by: his sons, Ryan Mayhew (Cassie) and William Mayhew (Kaitlin); his brother Michael Mayhew; his aunt Nancy Walker; his cousin Brooke Blazer; and his nieces, Lauren Setchel (Josh) and Lexi Adkins (Tyler). He is also survived by numerous other cousins, aunts, uncles & family members.
