He was predeceased by: his parents, LeRoy Henry Smith Jr. and Janice Mae Smith. He is survived by: his wife Gayle; his daughter Mackenzie (Ben); his grandchildren, Collins and Conrad; his daughters, Jordan (Andy) and Kellie (Jason); his grandchildren, Madison, Nathan and Colt; his son Daniel; and his grandchild Rhyatt.

In honor of Matt's memory, the family kindly asks that donations be made to the American Liver Foundation to support research and those affected by liver disease.
