He was predeceased by: his wife Brenda J. Blevins; his daughter Candace M. Blevins; his mother Lillian Collins; his son-in-law Darrell Wolfe; his granddaughter Stormie Kolle; and his brother Dewey "Eddie" Collins.
He is survived by: his children, Curtis Johnson (Kirsten Martinson), Nanette Davis (Bill) and Kristi Wolfe; his siblings, Jack Blevins (Susie) and Judy Duncan (Cecil); and his uncle Clarence "Pop" Blevins. He is also survived by seven grandchildren; 14 great-grandchildren; and a host of nieces, nephews, extended family, and friends.
