He is survived by: his girlfriend Angie Vealey; his children, Christopher David Cook (Megan) of Fairmont and Rachel Leann Moore of Danville; his granddaughters, Raegan Brooke Moore and Remi Max Moore; his siblings, Gary Cook (Carla) of Chesapeake, Ronnie Cook (Marcy) of Ottawa, Danny Cook (Georgann), Joyce Goodwin (Brad) of Ottawa, Vicky Guy of Raleigh and Jeannie Price (Timmy) of Raleigh; and his pet Nugget. He is also survived by nieces and nephews.

In lieu of flowers the family asks for donations to Saint Judes Children's Hospital in David's name.
