He was predeceased by: his wife Mary Ann Cook; and his parents, Joseph Cook and Minerva Cook. He is survived by: his sons, Mark Cook (Valory) and Joseph Cook (Teresa); his grandchildren, Steele Cook, Montana Cook, Josiah Cook (Erin) and Maddison Cook; and his siblings, Betty Jo Peery, Delores "Dot" Raines, Carol Sue Chandler and Kenneth Cook.
