He is survived by: his parents, Donald Samuels and Beverly Samuels; his children, Holden Samuels and Ashley Crawley (Cody); his brother Gary Samuels (Arleen); his brother-in-law Lynn Forstmann; his nieces and nephews, Kylie Felts (Jay), Courtney Forstmann (Paul) and Jacob Samuels; and his pet Gus.
