He was predeceased by: his parents, John Cook and Ethel Hensley Cook; his siblings, Julian "Jack" Cook, Lucian "Dick" Hensley and Delores Smith; and his siblings-in-law, Janice Cook, Pat Hensley and Jennings Smith.
He is survived by: his wife Patricia "Pat" Pape Cook; his daughters, Stephanie Cook and Jennifer Cook; his daughter-in-law Jennifer Barb (Muffy); his brother Donnie Cook; and his sister-in-law Ethel B. Cook. He is also survived by several nieces and a nephew.
