He was predeceased by: his parents, Andrew Mizak and Claudine Mizak; and his brothers, Steve and Mike Mizak. He is survived by: his children, Erick, Brian and Tammy (Enos); his grandchildren, Amber, Tristan and Jaxson; and his sister Cecilia Erickson.
