He was predeceased by: his parents, Thelma Ann Mauck and James Edward Smith; his son Kevin Duane Smith; his grandchildren, Elijah Edward Smith and Isabelle Rose Smith; and his sisters-in-law, Linda Lee York (Rutherford) and Regina Lou Gillispie (Cooke).
He is survived by: his wife Teresa "Terry" Gail Smith (Cooke); his son Stephen Preston Smith; his brothers, David Wayne York and Jeffery "Mutt" Smith; his mother-in-law Betty Lou Hudson (Reil); his sister-in-law Patricia Lee Green (Cooke); and his grandchildren, Caleb Preston Smith, Kyleigh Jade Smith, Khloe Alise Smith and Abigail Rae Smith. He is also survived by many nieces, nephews, cousins and friends.
In lieu of flowers, the family requests donations be made to the American Cancer Society or the American Lung Association.
