He was predeceased by: his parents, Jack Leslie, Sr. and Doris Whorley Johnson; and his siblings, Lou Skaggs and William C. Johnson. He is survived by: his wife Elizabeth "Libby" Allman Johnson; his daughter Dr. Larissa Johnson; his grandchildren, Josephine and Elias; his nieces, Kylene Casey (Andreas) and Andrea Fiegl (Charles); his nephew Josiah Johnson (Carlie); and his sisters-in-law, Lora Smith (Roy) and Linda Johnson. He is also survived by family members in addition to his wife.
