He was predeceased by: his parents, Bryan William Moak and Nolalee Horsley Moak Thomas; his brothers, Bryan "Skip" Moak and Bobby Ray Moak; his nephew Timothy "Tim" Moak; and his pets, Dimitri, Norman, Barney and Betty.
He is survived by: his wife Sheri Wichael Moak; his mother-in-law Gerry Wichael; his sister-in-law Kay Wichael; his brothers, Dennis Moak (Pam) and Paul Moak (LouAnn); and his son Andy Costa. He is also survived by numerous nieces and nephews, several great-nieces and nephews and four grandchildren.
