He is survived by: his wife Meina Sy Cook; his children, Holly Eve Cook, Timothy Cook and Cameron Cook (Lisa Cook); his grandchildren, Madison Cook and Riley Cook; and his sisters, Betty Ryan and Janette Frederick.
