He was predeceased by: his mother Eleanor H. Evans. He is survived by: his father John R. Evans, Sr.; and his siblings, Kimberly Jo Evans (Becky), John R. Evans, Jr. and Mark R. Evans (Patricia). He is also survived by numerous nieces, nephews and cousins.
In lieu of flowers, the family asks that memorial donations may be made in David's honor to the Hospice House in Williamsburg, VA, and to Gloucester Hospice.
