He was predeceased by: his parents, Raymond Smith Cook and Ruth Mary Bartlett; and his siblings, Raymond Smith Cook II and Maerian Delphine Baker (Cook). He is survived by: his wife Sandra Kay Cook; his children, Cathleen Viola Miller, Ruth Ann Cook, Daniel Brian Cook (Lisa) and Kevin Arthur Cook (Darlene); and his stepchildren, Terry Lyn Stillman (Dana), Randall James Biss, Robert Kevin Biss and Cary Daniel Biss (Trish). He is also survived by 14 Grandchildren, 17 Great Grandchildren and 2 Great-Great Grandchildren.
