He was predeceased by: his parents, Melvin R. Hazelwood and Pauline M. Hazelwood. He is survived by: his friend Buster; his wife Deby Hazelwood; his daughters, Niki Hazelwood (Scott Moore) and Paula Mireles (Eric); his grandchildren, Trey Cook (Tahlia), Ryan Cook (Chloe Morris) and Amelie Mireles; his great grandchild Chenoa Cook; and his brother Glenn D. Hazelwood (Brenda Wright). He is also survived by two nieces, one nephew, many cousins, and friends.
