He was predeceased by: his parents, James Forman and Mildred Spain Forman Arnold; and his brother-in-law Roger Charriere. He is survived by: his wife Michele; his sons, Austin Forman (Rachel) of Winchester, Virginia and Jacob Forman (Gui) of Washington, D. C.; his granddaughter Ryleigh Mae; his siblings, Gail Forman Carlton of Oxnard, California and Glenn Forman of Tappahannock, Virginia; and his sister-in-law Harlee Charriere of Key West. He is also survived by many nieces, nephews, and cousins.
