He was predeceased by: his parents, Stanley Polak and Dolores Polak (Wnenta); and his siblings, David Polak and Christina Polak. He is survived by: his wife Adele Degavage Polak of Manassas, VA; his daughters, Jessica Polak of Alexandria, VA and Virginia Myers (Zach) of Royal Oak, MI; and his brothers, Stanley Polak (Carol) of Houston, TX and Michael Polak (Margaret) of Manasquan, NJ.. He is also survived by many nieces, nephews, and extended family.
