He was predeceased by: his parents, Sam Slate and Bertha Slate; his brothers, Gene Slate and Doug Slate; and his daughter Pam Slate.
He is survived by: his wife Doris Slate; his daughters, Angie Slate-Graham (late Taylor) and Amy Anderson (Michael); and his grandchildren, Bailey Lea (Nick), Taylor-Christian Slate (Austin) and Hunter Chaney (Raina).
