He was predeceased by: his parents, Frank Costanza (Evelyn Costanza) and Evelyn Costanza. He is survived by: his wife Ashley; his siblings, Chris Costanza (Hope), Stacey Moore (Michael) and Dan; his sister-in-law Jen Batkin; his mother-in-law Sandy Batkin; his nieces, Kayla Costanza, Karlie Costanza, Elizabeth Moore and Lindsay Batkin; and his pet Blossom.
In lieu of flowers, donations in his memory can be made to the Divided Sky Foundation or Waterwheel Foundation, two worthy causes with connections to his Phish family, or to the Norfolk Animal Care Center.
